What version Joomla/CB/CB Antispam?

Did you install and publish it as a CB Plugin?
Is the antispam_captcha CB Field enabled and set for registration?

You likely have everyone configured to be CB Moderators and CB Moderators are exempt from Captcha. Navigate to CB > Configuration > Moderation and ensure "Moderator View Access Level" is strictly set (e.g. Special). If you've it set to Public (dangerous!) you have everyone being treated as CB Moderators.
