In many places the text "[formatname]" is showing where the username should be showing such as upon login,

This is also happening in all notifications that are sent out by CB
Seams like the formatname field isn't available. That needs to be publicly accessible as it's used for public name display. I'm guessing the tab it's on is maybe unpublished. You should find that field in CB > Field Management.
