CB Connect is already using several Open ID providers (e.g. Steam, PayPal, and Yahoo). It's possible to add more to it, but it would require modifying the plugin as it currently doesn't have support for general unknown providers. Are you needing to add a custom Open ID provider or is it a known provider already?

I'm working with a team who are making a medical app builder that handles login and compliance for the USA.
They don't want to build a community dev, we're hoping to use CB.
Is it an extensive code job, could we pay you guys to do it?
How will it impact updates into the future?
Depends on how you want this to work. Our current implementations are using buttons to link to providers. That requires you to provide the entire OpenID authentication backend to handle the token exchange. If all you really want to do is validate some credentials that's probably even easier by just making a Joomla authentication plugin (CB can utilize these) and they can type their username and password like normal. Since you would have control of both ends of things an authentication plugin is probably the more ideal route.
