For a reason or reasons not known by me, the CB Signup form on our veterans website now displays thus:
The privacy settings are supposed to be - and have consistently been - displayed contiguously with the appropriately assigned field.
Any ideas how to repair this?

Looks like you've simple privacy controls enabled which collects all the privacy controls into a single tab. Within CB Privacy > Parameters > Privacy > Simple you can toggle it off. It must've been enabled at some point (e.g. if you installed CB Quickstart that's the default behavior for CB Quickstart).
