I' m trying to configure the state drop down to dynamically update when the country is updated; the instructions say 'Configure the Update On parameter in Integrations > CB Core Fields Ajax to your previously created cb_usastate field' but the integrations tab does not show a CB Core Fields Ajax tab. I've just updated CB to the latest build and am using JOomla 3.9.18. What am I missing, please?

If you don't see CB Core Fields Ajax under the Integrations tab of your field then you need to install CB Core Fields Ajax plugin then be sure it's published. You should then see its parameters when editing your field.
