Ensure CB Privacy is public and published within CB > Plugin Management. If it is ensure you're not giving everyone moderator permission and set Moderator View Access Level within CB > Configuration > Moderation to something very strict (e.g. Super Users).
The Profile Privacy field also needs to be published and set to display on profile edit or registration. Note if you don't want to allow users to change this you need to set it to be moderator edit only under Integrations > CB Privacy. If the field is not set to display on profile edit or registration and isn't published it will not check it for privacy settings.
