Try moving it to a different tab. Canvas tab is an odd tab to place it on. Place it on the same tab as "Medical Expiry Date" and see if that resolves it. Aside from that I've no idea what's blocking it from displaying on registration beyond some sort of configuration issue somewhere. You can see this working fine on our demo site below.

Hi,
I'd already tried most of the tabs and ended up on the Canvas tab. I've put it back onto the contact info tab after trying the rest. I've also checked thorough the config on your site and set everything the same where possible but still no joy. I've found one third party Captcha that works (KeyCaptcha) but unfortunately it's a subscription one. I guess I'll have to keep looking or put up with the spam!

Are you sure you're using CBs registration and not Joomlas or a 3rd party registration page? CB AntiSpam registration captcha is only for CBs registration. If issue persists with CBs registration page please PM backend super administrator login credentials and will take a look.

ECC+ doesn't work after all - there's an issue with that as well.

Have reviewed your install. You've altered the core Special view access level and selected Public usergroup to be included in it. You've then set CBs Moderator View Access Level within CB > Configuration > Moderation to Special. This makes everyone a CB Moderator and CB Moderators are exempt from Captcha. This is why it isn't displaying. I've removed Public from the Special view access level within Users > View Access Levels and confirmed works fine now. Please be careful with ACL.
