I'm not seeing duplicate activity on your site. Please keep in mind it does not delete activity. So if you perform activity, revert, and perform it again you'll have 2 activity entries stored in database. You also can not use caching with dynamic content so if caching is enabled then purge it often, have short reasonable cache times (3 minutes or so), or disable it.
