There appears to be a coding problem with some recent extension updates, and I have not worked-out quite which yet, but likely to be Akeeba backup or Community Builder 1.4. One of them is looking for a table called jos_extensions, which only exists in J1.6. Adding this table to 1.5.22 fixes this problem, but there is some sloppy coding somewhere.
Unfortunately this kills 'Blue Flame Forms', so things are getting somewhat complicated to debug now-days!
