What is the Correct Excel format for Region, Country, and State

I am studying how to reproduce user’s location fields using CB Query (see attached image).

Your videos were very helpful.

Based on your demo, I understand there are 3 columns: (1) “Region”, (2) “Country”, and (3) “State”. 

However, the only information I am missing is the format of the Excel sheet to import the data to the database.

Is the format on the attached image correct?

That example is using the database provided by CB Paid Subscriptions. Below is an example using an independents database, but only supports US state and city.

What your database looks like is entirely up to you. There's thousands of examples across the web for such a database. Below is another example for a Country field that uses geonames.org data.
