Email title is set correctly at the field management for years, no it disappeared.
in sign up form field is showing but no text, see it here

Edit the email field within CB > Field Management and ensure "Show on Registration" is set to "Yes, 1 Line with Title" as looks like it maybe set to "Yes, 1 Line with Empty Title".
