Profile Photos vanished + wrong informations in user status

Hi!
I am facing a courious and disturbing problem in my cb 1.9 environement, on joomla 2.5 website. Profile's photos just disappeared and was replaced by default image of cb's pending image approval icon. Also User Status is not displaing correctly, for all users displays the same wrong informations:
- member since ... 3 months ago
- last time connected ... 3 days ago
- connections ... 12.
What happenned??? Did my account has hacked or is just something missing???
I tried to reinstall community builder but NO CHANGES!
I will greatly appreciate any possible help

Pending image approval means that user most likely uploaded a new image and your CB Configuration calls for moderator approval.

I see nothing wrong with a persone signing up to your site 3 months ago and logging in to your site for the last time 3 days ago.

I also see no issue with a person having 12 connections.

The problem is that ALL USERS have the same errors including me. It is not possible for all users to sign up in the same day 3 months ago, connecting to site 3 days ago and have 12 friends each, all the same, in the same time.
The real problem is this configuration changes every day for ALL USERS in the same time. For example, today, all users are connected, they are members since 2 months ago, last connected 5 days ago, connections 1. ALL OF THEM!!!
P.S. I never set moderator approval for profile's photo. Strange, very strange!!!

Any suggestions?

cb version?
was this a clean install or some kind of migration?

cb 1.9 environement, on joomla 2.5 website
was a clean install
Everything was ok until two weeks ago.
which is the script for avatar to search and display profile image and where can I make this modification? Perhaps something is wrong there
