Can you tell me when upgrading, are changes made to the database? What if I would like to go back to the prior version.

Depends on the version you are upgrading from.

There is no downgrading option - you should backup before upgrading just in case you need to rollback.

This is standard advice that you should apply whenever you are upgrading anything on your website.
