[SOLVED] Offline payment error- blank page

I get a blank page when recording a users offline payment in the front end.
I click Record offline payment link and it goes to a blank page.

In history log, it shows that payment is pending and member does not become activated.

please make sure you have latest CB 2.0.11 and CBSubs 4.0.1 releases and then turn on joomla debug and test again to see if anything more appears.

I have latest Joomla, CB and CBsubs, turned on joomla debug but problem is still there.

is joomla debug set to maximum level?
please also check error logs on your server.
