Looks like CB AntiSpam or the tab its on may not be accessible. Ensure CB AntiSpam is public and published within CB > Plugin Management. Ensure the tab the antispam_captcha field is on is public and published. Within CB > Configuration > Moderation ensure Moderator View Access Level is correctly configured (it should not be public, registered, etc.. it should be strict). If issue persists switch to default Joomla protostar template, clear Joomla cache, and retry. If still persists press F12 to open your browsers developer console, click the Console tab, and refresh the page to check for any fatal JS errors in the console (they'll be in red).
